To expedite your initial visit, please download the New Patient Forms in advance and bring them with you to your next appointment. You may also fax the completed forms before your visit. Please remember to bring your insurance cards, a valid driver’s license, and a list of current medications. Your insurance co-pay, if applicable, will be required at the time of your visit. If you are scheduled for a procedure, please remember to have someone drive you to and from the surgical center so you can be transported safely.
